Return Authorisation Process:

1. Please fill out and submit the form below to initiate a request for Return Authorisation.
2. Fields marked are required
3. Guardian will assign a Return Authorisation Number and email the Return Authorisation Form to the email address provided.
4. The shipping package, containing the returned product(s), must have the assigned Return Authorisation Number clearly marked on the outside together with the Return Authorisation Form.

All items returned to Guardian must have an officially issued Return Authorisation (RA) Number.

Guardian will NOT accept any returned items without a Return Authorisation Number.
